Checklist for Evaluation of New Member Applicants

NOTE: In order to be approved as a QMPC Member, applicant must satisfactorily pass ALL eligibility criteria

Regular Member

  1. Former LECC’s REGULAR MEMBER before the plant closure of on Nov. 30, 2008.

Associate Member

  1. Former LECC ASSOCIATE MEMBER before the plant closure of on Nov. 30, 2008.
  2. Permanent employee of QMPC
  3. Separated employee of QMPC with good employment record
  4. Regular employee of LSPI as of Nov 30, 2008 who was not a LECCI member (Aug 2008)
  5. FORMER LSPI employee and became a member of LECCI not falling to Regular Member
  6. FORMER LSPI employee and a non-member of LECCI not falling to Regular Member
  7. NOMINATED MEMBERS OF THE FAMILY
  • a. SINGLE – sibling/s and legitimate children (Children should be of legal age)
  • b. MARRIED – spouse and legitimate children only (Children should be of legal age)

ELIGIBILITY CRITERIA:

  1. Age requirement – 18 years old and above
  2. 100% Filipino Citizen
  3. With good employment record with LSPI and QMPC
  4. Good moral standing
  5. With no serious illness
  6. Stable source of income can be in a form of job, business or pension
  7. Not currently employed with LSPI or affiliates
  8. Was not terminated for cause from employment and from QMPC
  9. With NO labor case against LSPI before or after its closure and other external cases
  10. With NO labor or court cases against QMPC
  11. Was not a former QMPC member thrice (3 times) – Acceptable reasons – withdrawal, resignation and other valid reason.

NOTE: QMPC employee are qualified under Associate only, no voting rights and cannot be nominated on any position and cannot become regular members.
While New Associate Member will become a Regular Member once they meet GA Attendance and MIGS requirements for three consecutive years.

Required Documents for Submission

  1. Birth Certificate (applicant and dependents)
  2. Marriage Certificate (if married)
  3. Employment Certificate, Mayor’s Permit, or SSS Pension proof
  4. Barangay Clearance
  5. Medical Certificate
